Abstract:
Aim: Drug abuse is a major problem of contemporary society. Abuse of the codeine-glutethimide combination results in severe drug addiction, with high dependency, increased tolerance and a high mortality rate. The mechanism of interaction between codeine and glutethimide has not been fully elucidated. The main objective of this research project was to study this drug-interaction both from a toxic- and pharmacodynamic point of view, as well as from a pharmacokinetic point of view, in vivo and in vitro.
Methodology: 1) The toxic and pharmacodynamic interaction between codeine and glutethimide was evaluated experimentally in mouse by the isobole method and the thermal stimulation method. 2) A HPLC method for simultaneous quantification of codeine, morphine and morphine-3-glucuronide in rat plasma was developed based on their native fluorescence, after isolation by solid phase extraction (EFS). The optimized and validated method was applied in the study of the pharmacokinetic profile of codeine in rats and in the in vivo study of the interaction of codeine with glutetimide in rats. 3) The biotransformation of codeine to morphine was determined in vitro, on brain and rat liver homogenates, in the absence and presence of glutetimide.
Results: 1) A synergism between codeine and glutetimide was highlighted. Glutetimide prolonged and maintained the analgesic effect of codeine over time. 2) The association codeine - glutetimide led to an increase in codeine plasma levels and a decrease in codeine O-demethylation to morphine. Morphine plasma levels were significantly increased and remained at a relatively high level over a longer period of time, simultaneously with a significant decrease in morphine-3-glucuronide plasma levels. Therefore, glutetimide significantly inhibited glucuronidation of morphine. 3) In vitro biotransformation of codeine to morphine was studied in the liver homogenate. Glutetimide partially inhibited the CYP2D6 isoenzyme.
